STATEWIDE BIRTH CERTIFICATE ISSUANCE
Due to a large investment of resources by the Ohio Dept of Health, Geauga County residents born anywhere in Ohio after 1908 can now obtain certified birth certificates at the health district. Certified copies of birth records are available in our office or by mail for $23.00 each.Copies ordered online have additional processing fees. Please visit our Vital Statistics Web page

HAPPY BIRTHDAY WIC
The WIC program celebrated its 38th birthday on Sunday, January 15, 2012 when the first project opened in Pineville, Kentucky. Ohio’s program began a few weeks later. In that first year in 1974, Ohio had total funding in the amount of $757,340 and served 10,175 participants in 18 project sites in the state.
By 1984, Ohio WIC had grown to be statewide with the addition of the 88th county via the Geauga WIC program. In 2011, participation averaged 281,306 per month with an overall grant of $188.6 million and another $58 million in infant formula and infant foods rebates.
In 2011, Ohio WIC implemented a statewide breastfeeding peer helper program, provided nutrition education kiosks in all clinic sites, and continued meeting USDA development requirements for establishing electronic benefits transfer (EBT) with a 2015 target date.

insurance card. We are now able to accept Aetna, Anthem BCBS, Cigna, Humana, United Healthcare and
Medical Mutual of Ohio for seniors.
Most employer insurance plans are also accepted.
Visa, Mastercard and cash also accepted.
Pregnant Women must have a signed order from
the obstetrician.
