All births and deaths that have occurred since 1909 in Geauga County are registered with the Vital Statistics office of the Geauga County Health District.
Certifies copies of birth and death records are available in our office, by mail or online for $23.00 each. Only cash or check is accepted at the office. There are additional fees associated with online processing. Credit cards can be processed on line or by phone for an additional fee. Due to policies established by the State Department of Vital Statistics, we no longer provide uncertified copies..
To request a certificate either visit the office in person, or send your request with a self addressed stamped envelope and a check or money order payable to Geauga County Health District and mail to:

For birth certificates please include the name at birth and the date of birth.
For death certificates include the decedents name at death and the date of death.
