Order your Birth and Death Certificates Online here For your convenience, you can process online requests through an independent company that the Geauga County Health District has partnered with to provide you this service. VitalChek can be accessed through their Web site. An additional fee is charged by VitalChek for this service. All major credit cards are accepted, including American Express®, Discover®, Master Card®, and Visa®

All births and deaths that have occured since 1909 in Geauga County are registered with the Vital Statistic’s office of the Geauga County Health District.

Certifies copies of birth and death records are available in our office, by mail or online for $23.00 each. There are additional fees associated with online processing. Debit cards are accepted at the office and credit cards can be processed on line or by phone for an additional fee. Due to policies established by the State Department of Vital Statistics, we no longer provide uncertified copies..

To request a certificate either visit the office in person, or send your request with a self addressed stamped envelope and a check or money order payable to Geauga County Health District and mail to:

For birth certificates please include the name at birth and the date of birth.
For death certificates include the decedents name at death and the date of death (or approximate).
